Heart

Cardiology Specialist Nurses

Main remit

· Help with management of all STEMI and NSTEMI patients in the department

· Specialist assessment of suspected ACS presentation

· As an advice / review service in support of ED clinicians

· A liaison between ED and CCU / Cath lab

· External referrals and advice from GPs, MIUs etc

· Triage of all ED originated Rapid access Chest pain clinic referrals

Frailty Team

This team of experienced specialist nurses will see frail patients independently from the front door.

They can also provide advice and help in managing  patients with frailty in the department and those you may want to get home but could benefit from review.

JETT

The Jett Team are a combined team of physios and OTs who will see patients with:

-Frailty

-Falls, including patients in care homes.

-New changes to someone's functional ability or mobility.

-Patients struggling to manage at home.

-Patients with a new injury affecting their limbs (any age range).

-Patients who are the main carer for a dependent individual.

Child Therapist

CAHMS Team

The CAHMS team expect to see ALL patients under the age of 18 presenting with a mental health problem between 0900-2030. For out of hours referrals more information can be found on mphed.com. There is a risk matrix to complete and an email to CAMHS that needs to be sent for all patients. Again, see mphed.com for this.

Psychiatric Liaison Team

All adults (18 or over) with mental health concerns (who are fit for assessement) are suitable to be referred to our 24/7 Psych Liaison team. To refer, and to complete the matrix form, see mphed.com

Alcoholic Drinks

Somerset Drug and Alcohol Team (SDAS)

We have some excellent staff from SDAS who regularly work with us in the ED. If you have a patient who would benefit see mphed.com for more details.
