
Induction 6: Services available at MPH

Being a trauma unit, we have the capability of managing most cases on site. Exceptions include major poly-trauma, which once stabilised will be transferred to Southmead Hospital (or BRI for children), Cardiothoracic injuries (transferred to the BRI)|, majors burns or plastics required (usually Southmead Hospital).
Renal patients are managed at the RDE in Exeter but we have the capacity to dialysis in an emergency here at MPH.
All other sub-speciaties (including spinal, vascular, Max Fax and ENT) are managed here at MPH..
Specific Services commonly used by ED

Stroke
Stroke. Thrombolysis can occur 24/7 – in hours via stroke team, OOH via the medical registrar on call. Further info can be found on mphed.com
Thrombolysis Calls for those under 6 hours (we thrombolyse up to 4.5hours).
Thrombectomy can be discussed with the medical SPR on call.

Coronary Care
Acute MI. 24 hour access to cath lab for STEMI & Primary Cutaneous Intervention (PCI). If convincing cardiac ongoing chest pain even if NSTEMI may take to lab. Send ECG to Coronary care unit (CCU) and call to discuss with nurse in charge/ Cardiology consultant.
Time is critical.

Trauma (NOF) coordinator
Between 0900-1700 Monday - Friday the trauma co-ordinator will help to assess and manage patients with Neck of Femur (NOF) fracture directly.

ATMIST Calls
Ambulance service pre-alert for sick/concerning patients
Air vs. road ambulance – notify if helicopter as need porters to shut down traffic for safe landing.
Paperwork kept by phone:
ATMIST for trauma and serious medical cases
FAST for ? CVA patients – book CT, put out stroke or thrombolysis call.

Cauda Equina pathway – Book MRI Lumbar spine
